This log helps you give health-care providers an accurate history and keep medical and insurance records organized after a collision. It is not a substitute for medical care. Seek urgent help for emergency symptoms and follow advice from qualified providers.
Important Florida PIP timing
Florida PIP generally requires initial services and care from a qualifying provider within 14 days of a motor-vehicle accident for medical benefits. The rule, provider requirements, benefit levels, and exclusions appear in Florida Statute § 627.736. This page cannot determine whether particular care is covered.

Symptom and activity log
Use concrete observations. “Could not lift the laundry basket” is more useful than “felt terrible.” Do not guess at a diagnosis, copy the same entry every day, or exaggerate.
