Rideshare Accident Evidence Checklist

A focused Uber and Lyft accident checklist for preserving trip receipts, app status, driver and vehicle details, communications, and insurance records.

Print this page or save it as a PDF, then keep it with your accident records.

Rideshare claims often turn on one electronic fact: what the driver was doing in the app at the time of the collision. Save trip and app information before screens, links, or account details change. Also complete the general Accident Evidence Checklist.

Trip and app records

  • Electronic trip receipt showing origin, destination, time, distance, and fare
  • Trip or ride ID
  • Screenshot of the driver’s name and profile photograph
  • Vehicle make, model, color, and license plate
  • Pickup and destination locations
  • Route map and your location at impact
  • Screen showing whether the ride was requested, accepted, in progress, canceled, or completed
  • Estimated arrival and pickup times
  • Safety-center or incident-report confirmation
  • App support chat, emails, text messages, and push notifications
  • Any audio or video created through an in-app safety feature

If you were the passenger

  • Save the receipt outside the app as a PDF or screenshot.
  • Write down where you were seated and whether you were using a seat belt.
  • Record the rideshare vehicle’s impact points and every other involved vehicle.
  • Preserve messages sent to the driver before pickup or during the trip.
  • Identify the account holder if another person requested the ride for you.
  • Record the names and contact information of other passengers.

If you were the rideshare driver

  • Capture the exact app status and ride phase.
  • Save your personal declarations page and any rideshare endorsement.
  • Save proof of the coverage provided through the platform.
  • Preserve weekly statements, trip history, and work records if income is affected.
  • Download dashcam footage before it is overwritten.
  • Keep inspection, maintenance, and repair records for the vehicle.
  • Report the incident through the required platform and insurer channels; save every confirmation.

Why app status matters under Florida law

Florida Statute § 627.748 sets different insurance requirements when a driver is logged on and waiting for a request versus engaged in an accepted prearranged ride. The statute also requires the driver, upon request after a crash, to provide proof of the required coverage and disclose whether the driver was logged on or engaged in a ride.

Do not rely on the platform’s advertised maximum alone. Save the evidence needed to establish which coverage period applies, then review the actual policy and coverage response.
